View and complete your forms as a client

As a client, you can use the Client Portal Achieve Client App to view and complete your forms.

Desktop – Client Portal

  1. On the top menu, click My Profile.

  2. Click the Forms tab.

  3. You may have to browse the list, use the Search option, or filter the list to find the form.

  4. View or complete your forms:

    • To view a form:

      1. Click the form name or the View the form button to view the form.

    • To complete a form:

      1. Click the form name or the Fill in the form button to fill in the form.

      2. Fill out your answers in the required fields.

      3. Click Save.

Heading

Description

Form Name

The name of the form the client completed.

Source

The method through which the form was added to the client’s profile. The possible sources are:

  • Booking

  • Direct Link

  • New Purchase

  • Registration

  • Studio

Additional details about the source (like the service that was booked or the staff member who added the form) are also displayed.

Added On

The date and time the form was added to the client profile.

Status

The status of the form. The possible statuses are:

  • Completed

  • Not Completed

  • Not Required

Completed On

The date on which the form was completed.

IP Address

The IP address of the client who completed the form.

Action

In the Action menu, you can click the View the form button to view a completed form or click the Fill in the form button to fill out an incomplete form.

Mobile – Achieve Client App

  1. Tap the My Profile tab.

  2. Tap Forms.

  3. View or complete your forms:

    1. To view a form:

      1. In the list of forms, tap the form you want to view. The form is displayed.

    2. To complete a form:

      1. In the list of forms, tap the form you want to complete.

      2. Fill in the form as required.

      3. Tap Submit. The form is completed.
