When a business you’re visiting has a Self Registration Kiosk, you can easily register new profiles for yourself and your family members.
Note: After you register through the Self Registration Kiosk and set your password through the welcome email, you’ll receive another email informing you that your password changed.
Register as a new client from the Self-Registration Kiosk
To register as a new client from the Self-Registration Kiosk:
Click Register a new client.
Enter your profile details.
To register a related client, click Add Relationship at the top of the page, then enter their profile details.
To create separate login credentials for related client, set Personalized login credentials to ON.
To use the same phone number for the related client, set Use phone number to ON.
To use the same address for the related client, set Use address to ON.
Click Save.
If the business requires a waiver to complete registration, review the waiver, select the I agree to use Electronic Signatures checkbox, and provide your digital signature. The waiver will be duplicated to all family members’ accounts.
Click Save. Your registration is complete.
