To access your form templates, click the App Drawer button then click View All > Forms.
With Forms, you can easily create and modify customized forms to obtain client information.
Create a new form template
To create a new form template, follow these steps:
On the Form Builder page, click Add form.
Click the Untitled Form heading and give your form a unique title.
Build your form by dragging and dropping the field components from the left sidebar.
Click View variables to view a descriptive list of each variable that you can copy and paste into your forms. Variables copied into forms will generate useful strings of text for your forms when viewed by clients, such as dates and phone numbers.
Variables associated with purchases or services won’t work correctly on forms added to client profiles that aren’t tied to a purchase or service.
Credit card variables won’t work until after a purchase has been made.
Pasting tables into a Text component can cause some unexpected behavior in which the tables can’t be modified or removed from the form. For more information, see Troubleshooting: Tables aren’t functioning correctly when used in Forms.
Modify the fields you’ve added to your form, as required.
If you want a field to be mandatory for clients to complete, set the Required field option to ON.
You can move each form component up or down to change the order in which they will appear on the form. You can do this by either clicking the Up or Down arrows to move a component up or down one spot in the order, or by clicking the Move icon and dragging the component to the spot you’d like it to be.
To delete an added component, click the Delete button
Click Settings at the top of the page to configure the settings for your form. See Forms settings for more information.
At the top of the page, set the Accept responses option to ON if you want to start accepting responses from clients right away. Set this switch to OFF if you’re creating this form for future use.
Click Save.
Adding a new mandatory form for an existing service that clients have already attended will require these clients to complete the form the next time they participate in the service.
Modify an existing form template
To modify an existing form template, follow these steps:
On the Form Builder page, click the Action buttonfor the form you want to modify.
Click Edit template or Settings to modify the template components or the settings of your form, respectively.
Complete your modifications and click Save.
Duplicate a form
To duplicate a form, follow these steps:
On the Form Builder page, click the Action buttonfor the form you want to duplicate.
Click Duplicate.
A new form titled, “Copy of [your form’s name]” will appear in the list.
Only the form template and settings will be duplicated. Form responses won’t be copied over.
Delete a form template
To delete a form template, follow these steps:
On the Form Builder page, click the Action buttonfor the form you want to delete.
Click Delete.
If clients have responded to the form, you must delete the responses before you can delete the form. Otherwise, you will receive an error message that says, “form is in use and cannot be deleted.”
