Understanding Forms

Welcome to Forms, your tool for creating personalized forms that make connecting with your clients a breeze! Whether you’re starting a new business relationship, booking a service, or making a purchase, Forms has got you covered.

Forms let you design custom forms to gather essential information from your clients. You can set up your forms on your site to appear for clients before, during, or after they book a service or make a purchase. Even better, you can create unique forms for specific clients and send them directly using a special link. Here’s how it works:

  • Tailor forms for your business: Design custom forms for membership agreements, appointment notes, retreat waivers, and more.

  • Simplify client interaction: Easily prompt clients to fill out forms before, during, or after they book a service or make a purchase.

  • Personalize for individuals: Create unique forms and send them directly to one or more clients using a direct URL.

What can you do with Forms?

Here are some examples:

  • Manage agreements and waivers: Tailor legal documents to your services.

  • Sell unique products: Craft forms for special items, like personalized meal plans.

  • Conduct client satisfaction surveys: Get feedback to improve your services. (Video)

  • SOAP notes: Allows you to document continuing patient encounters in a structured way.

  • Intake forms: Collect essential information from individuals before they register for your services or before they participate in a specific service.

    • Registration forms: Collect basic details such as the client’s personal information, contact information, emergency contact, general health history, and consent and agreement.

    • Intake forms prior to a service: Customize the intake form to gather information relevant to the specific service the client is seeking. You can assess the client’s goals for the service, as well as develop PAR-Qs to assess clients’ readiness for physical activities.
