When creating or modifying a form, you can click the Settings button to access a list of customizable settings for that form. This article describes all the available settings.

When enabled, a numbering scheme will be applied to the components of your form.

Privacy for staff

Description

Display confidentiality disclaimer when opening this form

When enabled, staff members will be prompted with a confidentiality disclaimer message when opening this form from a client’s profile.

Allow staff members with permissions to amend this form after completion

When enabled, staff members with the Amend completed form responses staff role permission can edit responses to completed client forms.

Privacy for clients

Who should be able to complete the form?

This setting has two options:

  • When Client and staff is selected, both clients and staff members can fill out the form.

  • When Staff only is selected, clients won’t be able to fill out the form. A staff member with the appropriate permissions is required to complete the form for the client.

Who should be able to view the completed form?

This setting has two options:

  • When Client and staff is selected, both clients and staff members can view the form after it’s completed.

  • When Staff only is selected, completed forms will be hidden from client view in the Achieve Client App and Client Portal.

Form requirements

Client registration

Description

The client will be asked to fill in the form during registration

When enabled, the form will be displayed to clients for completion during registration.

Is it mandatory for the client to complete the form during the registration?

This setting will only appear if the The client will be asked to fill in the form during registration setting is set to ON.

This setting has two options:

  • When Form is optional is selected, the client can skip the form and won’t be required to fill it inbefore completing registration.

  • When Form is required is selected, the client will be required to fill out the form before completing registration. They won’t be able to book services, make purchases, or otherwise access your business through WellnessLiving until the form has been completed.

Service bookings

Description

The client will be asked to fill in the form during the booking process of the following services:

When enabled, the form will be displayed to clients for completion when booking the selected services.

This setting has two options:

  • When All services is selected, the client will be asked to fill out the form when booking any service.

  • When Selected services is selected, you can select any of your appointments, classes, events, and/or assets. The client will be asked to fill out the form when booking the selected services.

How many times should the client be asked to complete the form?

This setting will only appear if the The client will be asked to fill in the form during the booking process of the following services: setting is set to ON.

This setting has three options:

  • When Only once is selected, the client will be asked to fill out the form the first time they book any of the selected services. After that, the client won’t be shown the form when booking subsequent services.

  • When First booking for each service is selected, the client will be asked to fill out the form the first time they book each of the selected services. After that, the client won’t be shown the form when booking services that they’ve booked before.

  • When Every booking is selected, the client will be asked to fill out the form every time they book any of the selected services.

Is it mandatory for the client to complete the form during the booking process?

This setting will only appear if both of the following conditions are met:

  • The The client will be asked to fill in the form during the booking process of the following services: setting is set to ON.

  • The Who should be able to complete this form? option under PRIVACY FOR CLIENTS is set to Client and staff.

This setting has two options:

  • When Form is optional is selected, the client can skip the form and won’t be required to fill it out before completing their service booking.

  • When Form is required is selected, the client will be required to fill out the form before they can complete their service booking.

Store purchases

Description

The client will be asked to fill in the form during purchase

When enabled, the form will be displayed to clients for completion when purchasing the selected items.

This setting has two options:

  • When All items is selected, the client will be asked to fill out the form when purchasing any item from your business.

  • When Selected items is selected, you can select any of your Purchase Options, products, event purchases, and/or gift cards. The client will be asked to fill out the form when purchasing the selected items.

How many times should the client be asked to complete the form?

This setting will only appear if the The client will be asked to fill out the form when purchasing the following items: setting is set to ON.

This setting has three options:

  • When Only once is selected, the client will be asked to fill out the form the first time they buy any of the selected items. After that, the client won’t be shown the form when buying items from your business.

  • When First purchase only is selected, the client will be asked to fill out the form the first time they buy each of the selected items. After that, the client won’t be shown the form when buying items that they’ve purchased before.

  • When Every purchase is selected, the client will be asked to fill out the form every time they buy any of the selected items.

Is it mandatory for the client to complete the form when purchasing items?

This setting will only appear if:

  • The The client will be asked to fill out the form when purchasing the following items: setting is set to ON.

  • The Who should be able to complete this form? option under PRIVACY FOR CLIENTS is set to Client and staff.

  • When Form is optional is selected, the client can skip the form and won’t be required to fill it out before completing their purchase.

  • When Form is required is selected, the client will be required to fill out the form before they can complete their purchase.

Response options

Setting

Send response receipts to all clients

When enabled, clients will receive an email notification after submitting this form.

Send response receipts to staff members

When enabled, staff members with the staff role permission Receive response notifications will receive an email notification whenever a client completes this form.

Send response receipts to additional recipients

When enabled, email addresses you add to the Additional Recipients field will receive an email notification whenever this form is completed.

To add a recipient, enter their full email address in the Additional Recipients field and press Enter on your keyboard. Repeat this for each required recipient.

Require clients to sign in to complete this form

When enabled, clients must sign in to their WellnessLiving account before they can complete the form.

When disabled, clients who received the direct form URL can complete the form as a guest without signing in, and staff members can then associate those responses with the respective clients.

For form URLs sent via service reminders or notifications, form responses are automatically associated with the respective clients.

Send automated reminders for incomplete forms

When enabled, clients will receive a reminder notification if the form remains incomplete after the indicated time period (in hours or days).

Send response receipts to additional recipients

When enabled, allows you to add additional recipients who will receive a confirmation email each time the form is submitted.

Direct form URL

Provides you with a direct link to the form to share with clients that they can use to complete the form online. Clients will be redirected to your client portal home page after submission.

Kiosk mode URL

Provides you with a direct link to the form that will open in kiosk mode. A kiosk mode form is designed to run automatically on tablets, so that clients can fill it out at trade shows, studio open houses, or any activity in which clients can walk up to a tablet set up as a kiosk to fill out the form.

The advantage to kiosk mode is that the form flow will reset after a client completes a form, so your clients can complete the form back-to-back without staff needing to reset the form for the next client.
