You can add the side-by-side element to your forms to gives your clients the ability to upload two comparison images to show their progress over time, results from work done, and so on. For example, you can allow clients to upload a Before and After picture showing the effects of a specific workout routine or beauty treatment.

Add a side-by-side image element to a form template

To add a side-by-side image element to a form template, complete the following:

  1. To access Forms, click the App Drawer button then click View All > Forms.

  2. On the Form Builder page:

    1. To create a new form, click Add form.

    2. To edit an existing form, click the Action button then click Edit next to the form you want to modify.

  3. Under Field types, drag-and-drop the Side-by-Side element to your form template, then fill out the fields:

    • Set Required to ON if you want this field to be required by clients when filling out the form.

    • Under Heading, enter the question or prompt associated with the side-by-side element. This field is required.

    • Under Subheading, enter further details or information that the client may need when filling out this field.

    • Under Left image title and Right image title, enter the description that should appear on each image. By default, these descriptions are Before and After.

      • Click Upload image or Change image to upload a new default image for this field. This will appear to clients when filling out the form, and can be replaced with their own image.

  4. Click Save.

Fill out a side-by-side image element on a form

To fill out a side-by-side image element on a form, complete the following:

  1. Open the form:

    • To access a client's forms from the Staff Back Office:

      1. Search for the client using the Search button on the Top Nav Bar, then click on the client’s name to open their profile and navigate to Documents > Forms.

      2. Under the Action column, click the Action button next to the form you want to fill in, then click Fill in an incomplete form.

    • To access a client’s forms from the Elevate Staff App:

      1. Tap the Clients tab.

      2. In the search box, type the client's information, then tap the client's profile.

      3. Tap the Options button then tap Forms. Tap the client’s form you want to fill in.

    • To access your forms from the Client Portal:

      1. Click My Profile > Forms.

      2. Click the form you want to fill in.

    • To access your forms from the Achieve Client App:

      1. Tap My Profile > Forms.

      2. Tap the form you want to fill in.

  2. Upload your images:

    • Click or tap Upload image to add a new image, then upload your desired image.

    • Click or tap Change image to replace an image, then upload your desired image.

  3. Tap or click Save.
