Clients can sign waivers that are required for registration or service booking. A client can sign a waiver for themselves or on behalf of minors whose client profiles are linked as a relationship. A minor must have their First name, Last name, Email, Date of Birth and Home location completed on their profile before a waiver can be signed.
Sign an online waiver as a client
Follow the steps below on how a client can sign a waiver using the Self Registration Web App, Client Web App or Achieve Client App.
Desktop - Self Registration Web App
To sign an online wavier using the Self Registration Web App, follow these steps:
Click Find an existing client and enter your information in the box.
In the list of clients that is displayed, click the arrow
beside your name, then click Waiver.Click the checkbox labeled I agree to use Electronic Signatures.
Use your finger, stylus or a mouse to draw your signature on the line below Signature of participant and/or legal guardian/parent of participant 18 years old or younger.
Click Save. The waiver is now signed.
Desktop - Client Web App
If you're signing on behalf of a minor, click your profile icon in the top right corner of the screen. From the list, click the name of the family member whose account you want to access. You will be directed to that family member’s account.
To sign an online wavier using the Client Web App, follow these steps:
If you're not automatically prompted to sign the online waiver, click My Profile in the top menu, then click the Waiver tab.
Click the checkbox labeled I Agree to Use Electronic Signatures.
Use your mouse to draw your signature on the line below Signature of participant and/or legal guardian/parent of participant 18 years old or younger.
Click Submit. The waiver is now signed.
Mobile - Achieve Client App
If you're signing on behalf of a minor, tap the My Profile tab, then tap the profile icon at the top of the screen for the family member’s account you want to access. You will be directed to that family member’s account.
To sign an online wavier using the Achieve Client App, follow these steps:
If you’re not automatically prompted to sign the online waiver, tap the My Profile tab, then tap Waiver.
Use your finger to draw your signature on the line below Signature of participant and/or legal guardian/parent of participant 18 years old or younger.
Select the checkbox labeled I Agree to Use Electronic Signatures.
Tap Submit. The waiver is now signed.
View an online wavier as a client
Desktop - Client Web App
To view a wavier from the Client Web App, follow these steps:
Click the My Profile tab.
Click Waiver. The waiver will display.
Mobile - Achieve Client App
To view a wavier from the Achieve Client App, follow these steps:
Tap the My Profile tab.
Tap Waiver. The waiver will display.
