Viewing and managing form responses

To access your forms, click the App Drawer button then click View All > Forms.

From the Form Builder page, you can manage all the forms you have created, and manage client responses to them.

Manage client responses

To view and manage the responses your clients have provided for a form, follow these steps:

  1. On the Form Builder page, click the Action buttonnext to the form you would like to manage responses for.

  2. Click View all responses.

  3. Use the date selector, search bar, or filters to find the response(s) you’re looking for.

  4. Manage the response(s) as required:

    1. To view a client’s response in detail, click the Action buttonfor a response in the list, then click View Response. From here, you can also click Access log to view a log of all activity associated with this client’s response.

    2. To associate a response with another client, export responses to CSV or PDF, print a response, or delete a response, select the response(s) using the checkboxes on the left side of the table, then selected the actions you would like to perform using the menu on the bottom of the page.

Heading

Description

Form Name

The title of the form. You can click the title to access the form’s template.

Associated Services

The number of services associated with the form. You can click this number to access the form’s settings.

Associated Purchase Items

The number of purchased items associated with the form. You can click this number to access the form’s settings.

Last Response Date

The date of the most recent response to this form.

Number of Responses

The total number of responses to this form. You can click this number to access the form’s responses.

Accept Responses

Set this option to ON to allow users to submit responses to this form.

Set this option to OFF to stop users from submitting responses to this form.

Action

Click the Action buttonto access a menu from which you can edit the form, view responses to the form, copy a direct URL for the form, duplicate the form, or delete the form.

Description

Response Status

Use this filter to determine which forms to include based on whether the form is currently accepting responses.

This is determined by the toggle switch under Accept Responses for each form.

Heading

Description

Client Name

The name of the client who responded to the form. You can click their name to view their client profile.

Source

How the client accessed the form. For bookings, the name of the associated service, staff member, and session date will be listed.

Response Submitted Date

The date when the client’s response was submitted.

Response Submitted Time

The exact time when the response was submitted.

Action

Click the Action buttonto access a menu from which you can view the response in detail, export the response to either CSV or PDF, print the response, or delete the response.

Source

Use this filter to determine which responses to include based on source from which the client accessed the form.

Amended

Use this filter to determine which responses to include based on whether a response has been amended.
